Student Registration Process for
Branford Public Schools

PreK - 12

Welcome to Branford Public Schools!

Here is the step-by-step process to enroll your student for the 2022/2023 school year:

  1. Please have the following documents ready to upload to the online registration form:

    1. Proof of Age for the student

    2. Two forms of Branford, CT residency

    3. Most current health & immunization records

Documents*

* If you do not have any of these or similar documents or if your living situation classifies you as homeless, we can still assist you in enrolling for school. Please contact our central registration office (203-315-6723) so that we can assist you.

If the following documents have not been uploaded to the online registration form, parents/guardians can email the documents directly to the BPS Registrar, Susan Wallace at: swallace@branfordschools.org or if you prefer to submit them in person, call (203) 315-6723 for an appointment.
You can also fill out the online registration form on the computer in person in the Registrar's office at Branford High School.

Proof of Child's Age (any one):

  • Birth certificate or photocopy of birth certificate

  • Passport

  • Immunization records

  • Previous school records

  • Copy of custody agreement (if applicable)

Any two (2) of the following are acceptable forms of residency:

  • Bill from a bank, mortgage company, utility company, credit card company, doctor, or hospital

  • Bank statement or bank transaction receipt showing the bank’s name and mailing address

  • Pre-printed pay stub showing your employer’s name and address

  • Property or excise tax bill, or Social Security Administration or other pension or retirement annual benefits summary statement and dated within the previous 12 months

  • Current valid homeowner’s, renter’s policy, or motor vehicle insurance card or policy dated within the previous 12 months

  • Current valid Connecticut motor vehicle registration

  • Current motor vehicle loan statement for a motor vehicle registered in your name

  • Residential mortgage or similar loan contract, lease, or rental contract showing signatures from all parties needed to execute the agreement and dated within the previous 12 months

  • Connecticut voter registration card

  • Change-of-address confirmation from the United States Postal Service showing your prior and current address (Form CNL107)

Residency Affidavit

  • BPS accepts notarized documentation from parents or legal guardians residing full-time with a Branford Resident. The affidavit must be accompanied by all required documents for proof of residency. Contact Registrar for detailed information and form.

The Residency Documents MUST:

  • Show your name and your Branford residence address

  • Be dated within 90 days (unless stated otherwise below)

  • Be computer generated (typed)

Health & Immunization Records:
Submit your child's most recent physical & immunization records.

  • Use Connecticut’s Health Assessment Record Form (HAR-3) click here.

  • Make sure ALL pages of the Health Assessment Record are submitted:  

    • Part 1: Health History completed by parent/guardian

    • Part 2: Medical Evaluation (Physical) completed and signed by the Health Care Provider

    • Immunization Record: MUST list all immunizations 

  • Exam must be dated from September of current year to September of following year.

If Your Student is Transferring from Another School District:

  • Complete & Sign Release of Student Records Form

  • Must have a physical in the state of CT within a year of residency in Branford.

  • School Based Health Centers are located at Mary Murphy (MTM), Walsh Intermediate School (WIS), and Branford High School (BHS); health services are available for students who attend those schools. Click here for link on BPS website for SBHC.

If Your Student is Transferring from Outside the United States:

  • Submit student records from home country, and Branford Public Schools will translate them into English

  • Must have a physical in the United States

  • Submit health records from home country

  • School Based Health Centers are located at Mary Murphy (MTM), Walsh Intermediate School (WIS), and Branford High School (BHS); health services are available for students who attend those schools. Click here for link on BPS website for SBHC.

Before and After School Childcare:
Branford School Age Child Care (SACC) is available for students in Kindergarten through Grade 6 from the Family Resource Center/SACC Program. BPS Pre-Kindergarten students may also be eligible.

Your Student is Cleared to Start School WHEN:

  1. The online registration is approved by the Branford Public Schools Registrar

  2. The nurses have reviewed and cleared the student health records

  3. Transcripts from prior schools (transferring students only) have been reviewed and a schedule has been created by the guidance department (Walsh Intermediate and Branford High School only)
